After finding an appointment time that fits your schedule, you will be asked to register by creating a user name and password. After booking your appointment, you are able to access your pre-examination interview from home. Please be aware your appointment will not be confirmed until you complete the online registration and interview.  Personal Health Record (PHR) for your Eyes is a secure online portal you can conveniently access before your appointment to allow for a more efficient experience. If you are unable to complete, please arrive 10-15 minutes ahead of your scheduled exam time to allow us to assist you through the online interview process. If you do not cancel or reschedule your appointment with at least 24 hours notice, we may assess a $50 no show fee.

ESTABLISHED PATIENTS

Once you have logged in to your Personal Health Record for Your Eyes, you can book a new visit from the home screen. After booking your appointment, please complete your pre-examination interview. We look forward to seeing you again soon!
